What is case management?

CMSUK define Case Management as:

‘Case Management is a collaborative process which: assesses, plans, implements, co-ordinates, monitors and evaluates the options and services required to meet an individuals’ health, social care, educational and employment needs, using communication and available resources to promote quality cost effective outcomes.’

How can we help you as a client?

Our aim is for our clients to achieve the best outcomes possible and we recognise that this means something different to every client.

That is why we work in partnership with our clients to identify what their meaningful goals are and how we can support the client to achieve these goals.

Our service is therefore bespoke to each and every clients’ needs.

We offer highly skilled case managers to ensure that our clients are well supported and work with a person who they can develop a positive and trusting relationship with.

How can we help you as a solicitor?

We offer a comprehensive case management service which begins with a comprehensive assessment with detailed costings.

Following the assessment we can offer – 

  • recruitment, training and supervision of support workers using private recruitment or agency model
  • design, implementation and monitoring of care/support plans
  • a network of specialises therapists
  • coordination of the multi-disciplinary team
  • identifying appropriate properties and working with the team to ensure that appropriate adaptations are made
  • assistance with welfare benefits
  • client advocacy
  • working in partnership with education and other statutory services
  • family support and education
  • attendance at medico-legal meetings and conferences
  • witness statements, updated reports and court attendance
  • support worker payroll services
How do we keep your data confidential?

We work to a strict code of conduct to keep your information private. The information shared within your team will be kept confidential. We are required to share information with appropriate agencies or authorities if we are concerned about the well-being of someone (either yourself or someone else). In these cases, we would aim to let you know who we have spoken to and what has been shared.
